Well the pics prove a Yes

But for sfc i dont think an Exact model has been created though several kitbashes have been created. Fallen warrior or MP might have made one or 2 versions of this . One i did useing Jeff wallaces pretos ent era klinks in 2004 or 5 but i never released it due to way high poly count. The design is not hard to mock up it all depends on how much detail one wants. Mix up a Wz and Terradyhne works for this you got a rocking ship

One of atolm's pre-tos fleet has a ship that is close, but not exact... Hmmm

For starters it looks like a Lost era design.

I think the two necks are uncomplimentary, it looks like a hasty field repair job thrown together for a last stand somewhere. Klingon physiological redundancies aside; it just looks unfinished.

The design would benefit with armor over the two necks or better yet, a (marine, fighter bay) pod of sometype that would completely fill the space occupied by the two necks.

Just my two cents.

The answer is YES. Check Battleclinic IIRC there are models for all 3 SFC games, it may have been ported to BC as well.
