summary:

D2:
    * 2300-2400 (TCP&UDP), 47624(TCP) (Directplay 4)
D3:
    * 2302, 2303 ,6073 (UDP) (Directplay 8 )  <--- not 100% on these*
Gamespy:
    * 6667 (IRC)
    * 3783 (Voice Chat Port)
    * 27900 (Master Server UDP Heartbeat)
    * 28900 (Master Server List Request)
    * 29900 (GP Connection Manager)
    * 29901 (GP Search Manager)
    * 13139 (Custom UDP Pings)
    * 6515 (Dplay UDP) <--- hmmmm, MS says nothing about this....
    * 6500 (Query Port)

*check the  SFC3 network readme, but it has a misprint somewhere for one of the routers but is correct for the rest I know for sure 2303 is the correct one

allow pings from the wan too...

some are listed here: http://dynaverse.net/forum/index.php/topic,163349163.0.html  but not quite right...

some old directplay info here:
http://msdn.microsoft.com/library/default.asp?url=/archive/en-us/directx9_c_summer_03/directx/play/understand/networking/ports.asp

various game ports here:
http://www.u.arizona.edu/~trw/games/ports.htm

gamespy ports here:
http://www.gamespyarcade.com/support/firewalls.shtml

I trust your lan is static if you are going to use port forwarding?

There is more detail on the gamespy ports here on the forums somewhere (which are TDP and which are UDP, besides those labelled above)

Would also help if we knew what WIN version you're using and if you behind a router, etc.  ;)
But, FS has covered most of the bases....
