Crewmen who die problem.

Has anyone run into one or more of your crewman being injured during combat then never being able to gain more skill levels?  My security chief got injured, then after every battle so far it says that he has made a full recovery but no skill gained.  I even replaced him and it says the same thing for the new crewman.  Any ideas as to what is wrong and how I can fix this?  Thx
